CONGREGATE LIVING HEALTH FACILITIES (CLHF)
Congregate Living Health Facility – CLHF
A CLHF is a residential home that ranges from 6 to 18 beds that provide patient care to pediatrics and adults. The care rendered within a CLHF can be anything from medical supervision to 24 hour skilled care and other disciplines as ordered by the patients physician. Oftentimes, patients within a CLHF are developmentally delayed and / or have a Nasogastric tube, Gastric tube or tracheostomy that require constant care and supervision. Many of these patients are also Oxygen dependent.
Congregate Living Health Facilities (CLHF)
are defined as a residential home which provides 
inpatient care, including the following basic services: medical supervision, 24-hour skilled nursing and supportive care, pharmacy, dietary needs, social and recreational activities. The primary need of CLHF residents shall be for availability of skilled nursing care on a recurring, intermittent, extended, or continuous basis. The care is generally less intense than that provided in general acute care hospitals but more intense than that provided in skilled nursing facilities. Congregate living health facilities provide services to different types of patients:
persons who are mentally alert, physically disabled, who may be ventilator dependent
persons who are catastrophically and severely disabled. A catastrophically and severely disabled person is defined as a person whose origin of disability was acquired through trauma or a non-degenerative neurologic illness. It has been determined that active rehabilitation is beneficial for these types of individuals.
